Just to clear one doubt, may be wrong. 

Can it be because of pipe material?

May you try the consumable to weld a different/compatible base material, like SA106GrB, just on a sample piece with same welding procedure and do the radiography.

Just to confirm the issue is because of consumable itself.

Also may be the problem is because pf residual elements in the base material. Please check the MTC of pipe material.

Check/try the following:

As far as consumables are concerned:
  1. Bake (not preheat!) the electrodes, as per the manufacturer's recommended duration.
  2. Ensure the numbers 250, 100 what you have mentioned are not in ° F !! it should be ° C.
  3. If Vac-Pac electrodes are used, do not bake.
  4. Check baking oven, holding oven and quivers are properly calibrated.
  5. Limit the number of rebaking cycles to manufacturer recommendation.
  6. Check the shelf-life of electrodes with the manufacturer.
Others:
  1.  If pre heat is needed, before welding, check the pre heat of base metal is done properly, using approved procedures.
  2. Ensure the cleanliness of the area to be welded.(face, bevel, inside, outside for 1" minimum)
  3. Ensure sufficient wind protection is provided during welding.
  4. Ensure qualified welders are being used.
  5. Ensure approved WPS is being used.
Above mentioned are 'generalized' precautions only. If you provide some more data, it can be analyzed further.

SIR,

I'm engaged with welding of API 5LX 80  grade pipeline for regassified LNG transmission . We do  Tie-in method using SMAW+FCAW.
For root pass welding we use E7016-1 electrode after pre heating 250 degree for 2 hr in mother oven and maintain 100 degree in holding oven . during RT interpretation severe hollow bead and porosity observed in root .hence PMC sent almost all joints for repair.now it becomes a serious issue and I think ,the pre heat method was not proper. kindly advice me in this issue.
